I think their point is that old people keep going to clinics just because it’s cheap, and not because they actually need care.
It’s probably the same thing as city halls that have 相談 corners always being fully booked because everyone and their dog wants to 相談 just to have someone to talk to.
Obviously the elderly aren’t going to like it, but as a young person who has to pay the ridiculously high NHI I understand.
Makes sense. From a pure utilitarian point of view, spending a lot of money on a person’s treatments at the end of their healthspan yields to lower net well-being gains inevitably, compared to healthcare spent on younger people (e.g. curing the rare, relatively young cancer patients), vis-à-vis.
A big chunk of Japan’s tax money goes to health care for the elderly, a group that is growing all the time. It’s obviously not sustainable for a shrinking young population to keep financing that.
Doesn’t surprise me. Healthcare is incredibly expensive. The money has to come from somewhere and there are not enough young people to pay in.

Just FYI, Japan spends about 33% of the total national budget on social welfare. Out of which, about 50% goes to pension and long term care. That’s excluding healthcare expenditure for elderly care. So, about 17% of the national budget is spent on elderly people. Or more than 20% if you count in the elderly healthcare.
And that expenditure is ever growing. Politicians wanting to rollback a elderly care policy is a shitty situation for large portion of the population. But practically, there is just not enough money to sustain it.
Old people here has definitely turned local clinics to a type of community centers, and doctors play along and do random checks and follow ups to keep them coming back week after week. This is what they should be cracking down on.
I pay around 9万 a month, so would definitely feel a bit shitty if all the benefits disappeared once It’s my turn to get old/sick. Of course If I’m super well off close to my death bed then I can understand If I should fall into another category, which I assume is the case for a lot of older people today.
Finally someone is pushing for some meaningful fiscal reform. It is horrible but the country is going bankrupt and 50 years of crazy social cust structure choices have to be unraveled and it’s best the ones which enjoyed most the madness pay for it.
