TLDR: A bit of unusual case that has been going back for about a decade.
Dr Masahide Kanayama is a practicing Christian and alleged to have anointed 2 Japanese sites with oil for religious purposes. Japanese authorities considered to be a serious enough matter to extradite him.
He has also been practising medicine for about 30 years, more details about his background and this matter can be found here
The doctor and his lawyer have argued that he is being targeted by Japan for being Christian and that Christians have been repressed for centuries there.
If convicted, he faces up to 5 years in prison with hard labor.
His lawyer said he would ask the appeals court to stay its ruling until he could seek review by the U.S. Supreme Court.

5 years seems extreme for what, as far as the secular state should be concerned, basically little more than an insult.
That said, what an idiot

Incoming waste of taxpayer money. Bo-ho a man touched with oily fingers a piece of wood that some poor laborer placed in the 1950s during temple renovations. I can’t imagine all the money wasted by these cops the last 10 years trying to find him.

Very odd indeed. Seems like a vendetta or something to do with saving face(on the side of the Buddhist temple). I wonder whats the proof or I guess evidence required to be extradited…
