It’s asking for her registration number, along with her name (first name, last name).
If she’s paying the GEZ, she can look up her registration number – that’s what you need to fill in on the first line.
Rundfunkbeitrag is based on household, not individuals. Which means if she is already paying for the apartment then you don’t need to do anything. Even if you are registered there, you won’t probably receive any letters since the apartment is registered with her Beitragsnummer. But you can get reimbursed from overpaying your taxes from the previous apartment.
Don’t use services like dein-rundfunkbeitrag that wants to get payed the official website is free of charges, if your girlfriend doesn’t have pre payed option or use Kredit card for payment you will receive a letter with a payment invoice there is also a form to fill out that enables you to change the contract via snail mail.
