But how can this be the case and he also has the most mentions of anyone else in the files? Did they do a full AI redaction of his name, and then go through manually to un-redact where they were ok with him being mentioned? Perhaps this single instance was an oversight they forgot to remove?

They probably used a combination of wildcard and Boolean searches to narrow it down in the gov data set, then created review sets for each search term combination used.
Not sure what information retrieval capabilities Jmail uses, but worth a shot to confirm your findings.
