    1. > In its final report on the June 2023 disaster, which killed all five people on board, the agency said that the “engineering process for the Titan was inadequate” and “resulted in the construction of a carbon fiber composite pressure vessel that contained multiple anomalies and failed to meet necessary strength and durability requirements.”

      > The NTSB went on to say that OceanGate, the company that built Titan, also did not “adequately test” the submersible and was therefore unaware of its strength and durability.

      > The report also stated that the submersible’s pressure vessel likely sustained damage after it surfaced following its 80th dive, and it was then damaged further following its 82nd dive, which “further deteriorated and weakened the pressure vessel.”
